As per Suffolk County: Please ensure that the PDF version of these reports is fully downloaded and saved before entering any information into the document. We have also included all of the updated reports in Word format should you have an issue with the PDF format. The Word document should not be modified in any way. If any content has been altered, it will be returned.

•    Please note that although this report appears to be longer, the content remains the same. It is a structural change. You are still addressing the goals and writing a narrative on what you are working on and how the child is progressing. We gave 2 pages for goals, and we expanded boxes for typing which increases the number of pages. In the developmental domains, it is only necessary to write one to two sentences in each domain as your narrative will have discipline specific content.
