Policy On Mandatory Reporting of Domestic Violence

All Achieve Beyond staff are required to make a report if he/she knows or reasonably suspects a patient or the parent/guardian of a patient to be the victim of domestic abuse, suffering from a physical injury due to a firearm or assaultive or abusive conduct by themselves or a significant other.

The situation must be reported to local law enforcement via phone immediately. The phone number for the Whittier Police Department is 562-567-9200.

The witness should also make a written report to local law enforcement within 2 business days. The report should include the name of the injured person, the injured person’s whereabouts, the nature of the injuries, and, if known, the identity of the individual who inflicted the injury. A copy of the written report should be added to the patient’s medical record, if applicable. The report will be kept confidential and will only be shared with law enforcement and legal officials involved in the investigation or criminal enforcement of a criminal law implicated by the report.
