The standard quarterly progress note template for providers delivering Committee on Preschool Special Education related services in New York City. Documents the child’s present level of functioning and progress toward all IEP annual goals and short-term benchmarks each quarter using the standard Emerging, Ongoing, Mastered, and Not Yet Attained progress legend. Required for submission in December, March, June, and August.
