Pediatric occupational therapy is focused on helping clients and families engage and participate successfully in the daily activities that constitute a child’s well-being and development. Pediatric occupational therapists use play based and evidence based strategies to enhance learning and acquisition of new skills. Through evaluation and service provision, occupational therapists specialized in helping their clients function to the best of their abilities, despite identified disabilities. This may also include modification or adaptation strategies. In order to meet your child’s goals, occupational therapists will incorporate a variety of strategies based on your child’s individual needs. By identifying the skill deficit, the occupational therapist can then design a treatment plan to meet the long term goal. Daily activities/goals addressed may include:

  • Dressing
  • Feeding
  • Hygiene/Grooming
  • Toileting
  • Accessing the community/home
  • Developmental milestone attainment

Service Delivery Model: Achieve Beyond employs both licensed professionals and licensed/certified assistants. Our licensed professionals (OT, SLP, and PT) are qualified to perform the following services:

  • Administering evaluations/assessments
  • Treatment planning/implementation
  • Report writing
  • Creating/modifying goals
  • Direct service delivery

Our licensed/certified assistants (COTA, SLPA, PTA) each have a corresponding supervisor, who is employed by Achieve Beyond, and is responsible for all clients on their assistant’s caseload. Our licensed/certified assistants are qualified to perform the following services:

  • Implementing treatment plans from a licensed professional
  • Contributing to report writing
  • Summarizing goal progress
  • Direct service delivery

Initial Evaluations and Hours Recommendation: Our OTs, SLPs, and PTs complete assessments and will develop treatment plans that will be then implemented by either a licensed professional and/or a licensed/certified assistant. During this assessment, treatment hours will be recommended based on medically necessity and in conjunction with Achieve Beyond policies. These hours are based on each individual child, and research shows that the best outcomes are achieved when these treatment hours are delivered consistently. Conversely, a reduction in treatment hours may not produce the progress or outcomes that may be expected.

Achieve Beyond specializes in pediatric speech language therapy services that assist in the prevention, diagnosis and treatment of physical and/or cognitive difficulties relating to verbal and non-verbal communication. Speech pathologists (SLPs) assess and treat people with speech, language, voice, and fluency disorders, addressing both expressive and receptive language skills. Our licensed SLP therapists develop an individualized service plan tailored to each child’s needs. Deficits addressed may include:

  • Expressive and Receptive language
  • Articulation
  • Stuttering
  • Hearing impairments
  • Weak oral muscles

Achieve Beyond specializes in physical therapy services that help infants and children develop independence and safety skills allowing them to participate in their own environments including home, school and community settings. Physical Therapy is a field of science concerned with the ability and quality of motor skills or large motor activities related to movement. The patient referred to physical therapy may be disabled or delayed by illness, accident, birth defect or complications. A Physical Therapist’s (PT) role includes examination, evaluation, and client centered interventions toward achieving the highest functional outcomes for each patient/client. Areas addressed may include:

  • Muscle tone and strength
  • Posture/postural control
  • Pre-gait and gait training
  • Body alignment
  • Environmental adaptations/seating and positioning
  • Wheelchair positioning and mobility
  • Balance and Coordination
  • Increasing Range of motion
  • Endurance

ABA, known as Applied Behavior Analysis, is based on the science of learning and behavior. This science includes understandings about how behavior works and how learning takes place. ABA therapy applies these understandings to behavior treatments in a way that helps to increase useful or desired behaviors. ABA also applies these tenets to help reduce behaviors that may interfere with learning or behaviors that may harmful. ABA therapy is used to increase language and communication skills. It is also used to improve attention, focus, social skills, memory, and academics. ABA can be used to help decrease problem behaviors. The principles and methods of behavior analysis have been applied effectively in many circumstances to develop a wide range of skills in learners with and without disabilities.

  • ABA is considered an evidence-based “best” practice treatment by the US Surgeon General and by the American Psychological Association. “Evidence based” means that ABA has passed scientific tests of its usefulness, quality, and effectiveness.

  • ABA therapy includes many different techniques. All of these techniques focus on antecedents (what happens before a behavior occurs) and on consequences (what happens after the behavior). One technique is “positive reinforcement.” When a behavior is followed by something that is valued (a reinforcer), that behavior is more likely to be repeated. ABA uses positive reinforcement in a way that can be measured in order to help bring about meaningful behavior change.

  • A few types of interventions based on ABA principles include: DTI (Discrete Trial Instruction), NET (Natural Environment Teaching), Incidental Teaching, and Pivotal Response Training. Our language acquisition programs are based on Verbal Behavior (VB) methodology.

  • Research has shown that comprehensive ABA therapy, in which programs are implemented for multiple skill deficits, is most effective when implemented between 30-40 hours per week. Focused ABA therapy, targets only one or two deficits (e.g., problem behavior or social skills) is most effective with 10-25 hours per week of ABA therapy. These are guidelines, as the exact amount of therapy and level of parent involvement varies according to the specific needs of the child, and the BCBA doing the assessment will recommend hours based on their clinical opinion. While skills training programs are usually implemented by behavior therapists or teachers, parents are often taught critical skills to help their children transfer what they have learned in therapy to everyday life.

Information about our Program:

  • Delivery Model: As outlined by most insurance carriers, Achieve Beyond follows a tiered delivery service provision format where a Behavior Therapist (BT) or Registered Behavior Technician (RBT) provides the direct service and a Board Certified Behavior Analyst (BCBA) conducts the assessment, designs the treatment plan and supervises on a regular basis (at a minimum of 1 hour for every 10 hours of direct ABA).

  • Our BCBAs: are Masters or Doctoral level professionals who have completed 1500 supervised field hours, specific behavior analytic coursework and passed a competency exam. Our BCBA hold all required licenses and certifications.

  • Our Behavior Therapists: have undergone rigorous training in ABA. They have completed the RBT course work as outlined by the BACB. The BT is not responsible for making clinical treatment decisions. They consult with the BCBA before updating, modifying, or discontinuing any behavioral program. The BT is not a babysitter, teacher’s aide, shadow, or academic tutor. They are professionals who are responsible for enacting behavior analytic interventions as laid out in your child’s assessment and treatment plan.

  • Initial Assessments and Hour Recommendations: Our BCBAs complete assessments and will develop treatment plans that will be then implemented by the BTs. During this assessment, the BCBAs will recommend treatment hours that are medically necessary. These hours are based on each individual child, and research shows that the best outcomes are achieved when these treatment hours are delivered consistently. Conversely, a reduction in treatment hours may not produce the progress or outcomes that may be expected.

  • BCBA Supervision: takes several forms.

    • Direct Supervision: is provided in person which involves the BCBA observing and supervising the BT implementing the treatment plan. This includes: making any necessary revisions to program goals, monitoring treatment integrity and guiding staff and caregivers in running the treatment protocols.

    • Indirect supervision is off-site and includes developing the treatment goals, materials, protocols, summarizing and analyzing data, and coordination of care.

    • Family training consists of working with caregivers on implementing behavior analytic strategies and teaching concepts of ABA. If you have any questions about these roles, please ask your BCBA.

  • Pairing: The pairing process at the onset of therapy is an essential part of ABA programming. It enables the BT to develop a positive relationship by building rapport with your child. In this initial phase of programming, the BT will not be presenting any instructions—in most cases it will look like play. Once your child trusts that the BT will deliver good things after they respond correctly/appropriately, the foundation has been set to begin teaching skills. You may want the team to dive right into instruction, however, skipping this step may hinder future progress and compliance. It should be noted that the time it takes for the BT to be “paired” varies greatly by child.

  • Behavior Intervention Plans: A BCBA may devise a BIP to address challenging behaviors. BCBAs should review all facets of the BIP with you before implementation. BTs will be trained on the protocol during supervision sessions.

  • It is important that you understand and accept all procedures in order for the program to be successful. If you feel as though a plan is too difficult or effortful to manage, please inform your BCBA so they can provide you support and make any appropriate and necessary modifications.

  • Parent Participation: Parent participation is an essential component to effective and long-lasting behavior change. Parents/guardians are required by the insurers to be active participants in therapy.

A little information about our services and policies…

  • Insurance Guidelines: Our program is primarily funded through commercial insurance companies. As such, we need to follow certain regulations specific to each company. This is why we need accurate data and graphs (ABA), re-assessment and progress reports, and to stay within the guidelines of the treatment plan and service hours.
    • If your insurance changes or you change employers, you must notify the office immediately! We will have to get a new authorization and services may be put on hold until a new authorization is secured. If you do not notify us of a change in insurance, you may be required to pay out of pocket for the services.
  • Cancellations: We understand that there are times when you must cancel a session due to illness or emergencies. Cancellations should be made at least 24 hours before your session whenever possible. The therapist should be contacted as soon as possible. Makeups should be scheduled at the convenience of the family and the therapist.
    • Should your child have a contagious illness which consists of, but is not limited to, fever, vomiting, flu, rash, lice, eye infection, strep throat, and/or diarrhea, the scheduled session must be canceled for 24 hours from when the symptoms stopped. Children with open wounds, cuts, sores, and infections should be kept at home and have the sessions cancelled and/or rescheduled. Your therapist will have a copy of the assessment and should be aware of any medical conditions or alerts, if these conditions were noted and documented during the assessment. Please go over this area of the assessment with your therapist to ensure that all your health/medical concerns have been noted.
    • We also ask that during home-based therapy, if a member of the household has a contagious illness, that the therapist is informed prior to the session so that they may use their discretion to determine if the session should occur.
  • Clinic Cancellations: Please be aware that Achieve Beyond has a strict cancellation policy. For cancellations made at least 24 hours before the scheduled session, pending therapist availability, we will try to provide make-ups as soon as possible. However, if it is noticed that there are an excessive amount of cancellations (3 or more within 6 calendar months), your child’s case will be reviewed to see if a discharge is warranted. A “No-Show” is either when a client does not show up for a scheduled appointment (giving no prior notice of absence to the office/therapist) or, when a client notifies office/therapist less than 24 hours before a scheduled appointment. After 3 ‘No Show’ appointments (within 6 calendar months), the child’s case will be reviewed to see if a discharge is warranted.
  • Medications and allergies: If your child has a medical condition or severe allergy that may require immediate medical attention, such as an EpiPen, an adult caregiver must remain on-site while services are being rendered at all times. Medications will never be administered by an Achieve Beyond employee.
  • Session notes: All session notes must be completed with the date and time when you sign. Once you sign a session note, you agree to the times, date, and services listed. It is acceptable to have caretakers sign for the sessions provided, as long as they are over the age of 18 and you signed and returned the informed consent form.
  • Data and Debriefing: We advise therapists to take the last 10-15 minutes of every session to transcribe any data collected into the program book and to plot any data points for that day (for ABA therapists), complete the session note, and debrief the family. This is an integral part of therapy. The therapist will also speak with you about the session during this time.
  • Authorization Periods: Once you receive an authorization, your child is given a certain amount of hours for the specified period. This has been determined by the evaluator/assessor (SLP, OT, PT, or BCBA) who assessed your child, using their clinical judgement based on medical necessity. These hours should be used on a consistent schedule, and do not have anything to do with the school calendar. Makeups should always be brought up to your therapists and administrative staff. Please note that school holidays do not always apply to the clinic. If you would like makeups, but your therapist cannot provide them, please contact the office so we can try to add another therapist to your case.
  • Incident Reporting: When an accident/emergency/injury or other incident occurs, the therapist will complete a Special Incident Report Form within 24 hours of the incident and submit it to the office. Additional notification may be required for certain funding sources. Documentation of any such incident will be kept at the office in the client’s confidential file.
  • Mandated Reporting: Achieve Beyond staff members are mandated reporters and are required by law to immediately report any indications of suspected child abuse and/or neglect before, during, or after the course of any session.

 

In-Home Services:

  • Achieve Beyond therapists are encouraged to use toys and materials within the child’s natural environment and not to bring materials. If your child is not motivated by his or her toys because (s)he has free access to them, it may help to designate certain “special” toys for use with the BT only. Materials for the Behavior Intervention Plan (BIP) should be purchased by the family and not the behavior therapist.
  • A parent or guardian over the age of 18 must be present in the home for the entire duration of the session.
  • Achieve Beyond will never be able to drive your child.
  • We request a designated area for therapy to be conducted in your home. The area should contain a table and chairs and the necessary learning materials that will be communicated by your BCBA. Due to the nature of teaching through motivation, we also ask that you have all preferred items/activities readily available for the therapist to use in order to teach using positive reinforcement.
  • We want to make the therapeutic environment comfortable for all involved. Your provider will communicate with you regarding any temperature changes, distraction level, or any other possible factors that can contribute to the most appropriate learning environment.

 

In-Clinic Services:

  • Achieve Beyond will provide the necessary learning materials for your child within our clinic setting. We require that any edible reinforcers are brought in by the families. If your child is scheduled to stay throughout the day (e.g., morning through late afternoon), it is mandatory that you provide a packed lunch and drink for your child.
  • Upon arrival to the clinic you must sign in and wait in the waiting room until a clinician greets you. Children must attended to by a parent/guardian when they are not in session.
  • Children attending the clinic must be fully clothed and wearing shoes (shoes/socks may be removed at the request of the therapist while accessing gym equipment).
  • Parents are allowed in designated areas only, in order to be in compliance with the Health Insurance Portability and Accountability Act (HIPAA). These areas include the lobby, waiting room, and clinic rooms (when accompanied by staff).
