Job Summary:

Under the Case Manager’s supervision, the Case Coordinator will monitor and coordinate provision of Early Intervention therapeutic services to children with developmental delays.

Essential Duties and Responsibilities:
  • Support therapists and families and ensure that Early Intervention services are provided in accordance with NY State and EI Program rules and regulations
  • Process newly approved service authorizations and provide therapists with necessary information and documentation
  • Help providers establish contact with parents and assist with scheduling of therapy sessions
  • Coordinate with therapists, parents, and Service Coordinators to facilitate timely and consistent provision of services 
  • Communicate with pediatricians and medical personnel to obtain prescriptions and medical clearance when needed
  • Follow up with service providers to request and obtain progress reports in a timely manner
  • Follow up with service providers, service coordinators, and parents to obtain necessary documentation and consent forms
  • Conduct parent surveys on a regular basis to ensure parents’ satisfaction with services 
  • Document all information that pertains to the child’s Early Intervention services in the case activity log
  • Update the company’s database and ensure it contains accurate information
  • Maintain positive professional relationship with Service Coordinators and service providers
  • Respond to e-mail and phone inquiries from Service Coordinators, therapists, and office staff promptly
  • Collaborate with other departments, such as Staffing, Provider Relations, Billing, etc.
  • Other duties as assigned by the manager

Qualifications and Other Requirements:
  • Excellent verbal, written, and interpersonal communication skills 
  • Fluency in Spanish required
  • Strong organizational, time-management, and analytical skills
  • Proficient in Microsoft Office (Word, Excel, Outlook)
  • Ability to work in a fast-paced environment
  • Bachelor’s Degree Preferred
Physical Requirements:
  • Occasionally ascends/descends stairs going to, while at, or leaving work location or facilities traveled to for work events.
  • Must be able to remain in a stationary position for prolonged periods of time up to 90% of the work day working on a computer/electronic device.
  • The ability to observe details at close range (within a few feet of the observer).
  • The ability to communicate information and ideas so others will understand. Must be able to exchange accurate information in these situations.
