OSC Progress Report Guidance (please do not modify the progress report template.)

OSC progress reports are no longer required for every IFSP meeting. However, they should be completed in the following circumstances, where the information is critical to continuity of services, effective planning, and the necessary APR data that the state mandates from counties:

1)    IFSP Meetings Nearing Transition
When an IFSP meeting occurs close to a child’s transition, the OSC report should document:
       •    Whether transition paperwork has been completed with the family
       •    Whether the family has registered with the school district
       •    Whether CPSE evaluations have taken place
       •    Whether a CPSE meeting has been scheduled

2)    IFSP Reviews with Late or Interrupted Services
For IFSP reviews where services were delayed or there were gaps in service delivery, documentation should include:

       •    Reason for any service delay and staffing issues
       •    Steps being taken to address and resolve these concerns
