Job Summary:

We are looking to hire a  Service Coordination Supervisor/ SC to join our growing Forest Hills NY team. The Service Coordination Supervisor/ SC is responsible for ensuring that the department operates in full compliance with all policies and procedures established by New York State, the New York City Department of Health, and other governing regulatory agencies. In addition to managing an assigned caseload, the Supervisor oversees departmental performance to ensure that services are delivered effectively and in accordance with quality standards both in the department and for their own cases. This position requires experience as a Service Coordinator in the NYC Early Intervention Program.

$67,000.00 - $70,000.00 annually

Essential Duties and Responsibilities:

Supervisor Responsibilities:

  • Stay current on regulations and guide the department accordingly.
  • Train, coach, and provide feedback to Service Coordinators (SCs).
  • Assign cases weekly and monitor caseload transfers.
  • Oversee compliance with EI timelines, productivity, and documentation.
  • Run and analyze reports, present performance trends.
  • Audit cases (weekly closings and monthly random reviews).
  • Provide technical support and resolve complex cases.
  • Collaborate with providers, families, city officials, billing, and other departments.
  • Perform other duties as assigned.by manager

Service Coordinator Responsibilities:

  • Manage an EI caseload (minimum 20 cases)
  • Follow up promptly on assigned cases and monitor ongoing services.
  • Coordinate evaluations and therapies (Speech, PT, OT, etc).
  • Support and educate families, including advocacy and transition planning.
  • Ensure timely documentation and compliance with NYS/NYC rules & regulations.
  • Maintain accurate records in web portals and databases.
  • Conduct fieldwork (home or office visits) as needed.
  • Attend monthly supervision meetings.
  • Perform other duties as assigned.by manager

Requirements:
  • Must have at least 4-5 years experience as an NYC Early Intervention Service Coordinator
  • Must have a Bachelor’s degree in a health or human service field.
  • Must have at least 2-3 years experience as an EI SC Supervisor
  • Self-motivated individual who will be able to work independently.
  • Excellent organizational and communication skills.
  • Can multi-task and meet multiple deadlines.
  • Must have the set-up to work from home if and when needed.
  • Willing to travel around New York City for cases
  • Bilingual Spanish a plus
Physical Requirements:
  • Occasionally ascends/descends stairs going to, while at, or leaving work location or facilities traveled to for work events.
  • Occasionally moving and/or lifting objects at work weighing up to 20 pounds such as files or copy paper.
  • Must be able to remain in a stationary position for prolonged periods of time up to 90% of the work day working on a computer/electronic device.
  • The ability to observe details at close range (within a few feet of the observer).
  • Ability to read extensive material/documentation.
  • The ability to communicate information and ideas so others will understand. Must be able to exchange accurate information in these situations.
Compensation: $67,000-$70,000 Annual Salary
