Achieve Beyond is a national Pediatric Therapy and Autism Services agency. With offices in New York (Metro area/Long Island, Hudson Valley & Albany), New Jersey, Virginia/DC area, Delaware, Connecticut, Maryland, and California (greater Los Angeles area, the greater San Francisco area, San Diego, and Orange County). Achieve Beyond provides quality evaluation and therapy services such as special education, autism services, speech, physical, and occupational therapies to children birth through twenty-one years of age via telehealth, in clinic and in homes and the community.

Under the Case Manager's supervision, the Case Coordinator will monitor and coordinate provision of Early Intervention therapeutic services to children with developmental delays

Responsibilities:

Case Coordinator:

  • Maintaining the assigned caseload and conducting follow up on a weekly basis
  • Providing therapists with necessary information and documentation
  • Making sure providers initiate therapy in a timely manner
  • Requesting medical prescriptions and service recommendation forms
  • Requesting and obtaining progress reports before due dates in a timely manner
  • Documenting all activities in the activity log in a clear and concise manner
  • Updating internal database and making sure it contains accurate information
  • Responding to e-mail and phone inquiries from Service Coordinators, therapists, and office staff promptly
  • Maintaining a positive professional relationship with Service Coordinators and service providers
  • Conducting parent surveys
  • Provide coverage of case managers and receptionist phone during lunch breaks and/or office absence
  • Performing other administrative duties as needed

Requirements:

  • Bachelor's Degree in Psychology or related field
  • Experience in the Early Intervention Program or a similar social service setting preferred
  • Excellent verbal, written, and interpersonal communication skills
  • Strong organizational, time-management, and analytical skills
  • Ability to work as part of a team
  • Ability to work in a fast-paced work environment
  • Knowledge of MS Excel, Word, Outlook, and customized databases
  • Fluency in Spanish a plus
