Achieve Beyond is a national Pediatric Therapy and Autism Services agency. With offices in New York (Metro area/Long Island, Hudson Valley & Albany), New Jersey, Virginia/DC area, Connecticut, Minnesota, Maryland, Delaware, and California (greater Los Angeles & Orange County, and San Diego). Achieve Beyond provides quality evaluation and therapy services such as special education, autism services, speech, physical, and occupational therapies to children birth through twenty-one years of age via telehealth, in clinic and in homes and the community.

An Ongoing Service Coordinator (OSC) helps the families of children (birth to three years old) with developmental delays navigate all aspects of the Early Intervention Program (EIP). The OSC is responsible for coordinating services across agencies and assists in the development of Individualized Family Service Plans (IFSPs). The OSC is the main contact for the family; this includes providing regular follow up of child’s services, resolving conflicts between caregiver and providers when necessary, and ensuring the team of providers is adhering to the IFSP. The OSC provides families with information on available services, resources and options based on the needs of the child and the family. When appropriate and based on reason of discharge, the OSC prepares the family with all appropriate transition options available to the family after leaving the EIP. As the EIP is heavily regulated, the OSC ensures that every child’s record is always audit-ready and extensively documented.

  • Monitoring ongoing services monthly for a number of children including, but not limited to: Special Education, Speech Therapy, Physical Therapy, Occupational Therapy and Autism Services.
  • Conducting timely follow-up with families from the assignment of a case.
  • Assisting families in identifying available evaluation providers or service providers.
  • Ensuring timelines of all paperwork submission and implementation of New York State and Nassau/Suffolk County regulations and policies.
  • Informing families of advocacy services, when applicable.
  • Facilitating the development of a transition plan to preschool services, if appropriate.
  • Providing organization and management of all child records.
  • Navigating NYEIS (New York Early Intervention System) by updating and uploading necessary information and documents for every child.
  • Conducting home visits and reviews in the office or in the field.
  • Documenting all activities on a daily basis and meeting the weekly productivity requirement.
  • Attending weekly supervision meetings for training and development purposes for the first two months, thereafter will be monthly and/or as needed and requested by the supervisor.
  • Attending monthly department meetings for training and development purposes.
  • Completing a total of 10 annual training hours as mandated by New York State.
  • Other duties as assigned by manager


  • Bachelor's Degree in Psychology or a related field (Social Work, Sociology, Early Childhood Education, etc) required
  • Experience working with children and/or families (even internship experience) preferred, but not required
  • Must be willing to regularly travel within Nassau and/or Suffolk County (with a valid drivers license). The position requires time spent in office and in the field.
  • Bilingual Spanish required.
  • Should maintain good interpersonal and leadership skills. The position requires being the point person between county representatives, service providers, and families.
  • Demonstrates excellent time management and organizational skills to meet the various deadlines mandated by county, state, and agency. The position must follow strict state and county regulations, so specific procedures are in place to ensure compliance and confidentiality.
  • Should have strong writing skills. The position requires thorough documentation of all activities conducted.
  • Should be technologically savvy. The position utilizes various electronic programs for documentation and maintenance of child records.
  • Should be culturally competent and comfortable working with families with a variety of backgrounds and means.

Physical Requirements:

  • Must be able to travel to and from assigned cases including but not limited to driving personal vehicle, walking, and/or use of public transportation such as a bus or train.
  • The ability to climb and descend staircases.
