REPORTING INCIDENTS OF NON-COMPLIANCE

All Staff Members MUST Report all incidents of Non-Compliance to the Compliance Officer, a Member of the Compliance Committee or their respective Program Manager.

If you are unsure whether or not an incident is actually an incident of non compliance, report it anyway. IF YOU SEE SOMETHING, SAY SOMETHING!

Reports may be made in person, in writing, over the phone or online via the Consumer Report Form.

In your report, fully describe the incident and all parties involved.

All Reports will be kept Confidential and may be shared with Company’s Legal Counsel.

There will be no retaliation or adverse consequences for reporting in good faith.
