Case Coordinator

SUMMARY/ JOB OBJECTIVE:

We are a mid-sized national Healthcare agency based in Forest Hills, Queens with a diverse population of employees. We are currently seeking a Case Coordinator. Under the Case Manager’s supervision, the Case Coordinator will monitor and coordinate provision of Early Intervention therapeutic services to children with developmental delays. The responsibilities will include, but are not limited to, the following:

RESPONSIBILITIES:

  1. Maintain the assigned caseload and conduct follow up on a weekly basis
  2. Provide therapists with necessary information and documentation
  3. Make sure providers initiate therapy in a timely manner
  4. Request medical prescriptions for certain types of therapy
  5. Request and obtain progress reports in a timely manner
  6. Conduct parent surveys
  7. Document all activities in the activity log
  8. Update database and make sure it contains accurate information
  9. Respond to e-mail and phone inquiries from Service Coordinators, therapists, and office staff promptly
  10. Maintain a close professional relationship with Service Coordinators and service providers
  11. Perform other administrative duties as needed

REQUIREMENTS:

  • Bachelor’s Degree Required.
  • Experience in the Early Intervention Program or a similar social service setting preferred
  • Excellent verbal, written, and interpersonal communication skills
  • Strong organizational, time-management, and analytical skills
  • Ability to work as part of a team
  • Ability to work in a fast-paced work environment
  • Knowledge of MS Excel, Word, Outlook, and customized databases
  • Fluency in Spanish a plus
