Ongoing Service Coordination

We are a mid-sized national Healthcare agency focused on serving children and families with special needs and developmental delays. We are currently seeking an Ongoing Service Coordinator for Nassau and/or Suffolk Counties to assist families in the Early Intervention (age’s birth to 3) program. Ongoing Service Coordinators work closely with County Officials as well as families to ensure the children’s needs are met including evaluations and ongoing services. We are looking for a self-starter, motivated individual with the ability to multitask and the desire to learn and grow. This position requires analytical strength and ability to work as part of a team. We prefer a candidate with relevant industry background but are willing to provide training.


  1. Monitor ongoing services for a number of children including:
    • Special Education
    • Speech Therapy
    • Physical Therapy
    • Occupational Therapy
    • Autism Services
  2. Assisting families in identifying available service providers
  3. Assure timeliness of all paperwork submission and assure implementation of NYS and County Regulations and Policy
  4. Informing families of advocacy services
  5. Coordinating with medical and health providers
  6. Facilitating the development of a transition plan to the preschool, if appropriate.
  7. Organization and management of all child records


All early intervention service coordinators should meet the following qualifications. A minimum of one of the following educational or service coordination experience credentials:

  1. Two years experience in service coordination activities as voluntary or part-time experience which can be verified will be accepted on a pro-rata basis; or
  2. One year of service coordination experience and an additional year of experience in a service setting with infants and toddlers with developmental delays or disabilities; or
  3. One year of service coordination experience and an Associates degree in a health or human service field; or
  4. A Bachelor's degree in a health or human service field.

LANGUAGE SKILLS: Fluency in spoken and written Spanish is required.

TECHNICAL SKILLS: Basic computer skills are required as well as knowledge of Microsoft Office programs.

TRANSPORTATION: Must have valid drivers license and access to a car
