office in the greater Chicago, Illinois (IL) area,the greater Los Angeles, California (CA) area, Northern Virginia (VA) / Maryland / DC area Connecticut (CT) or New York (NY) for a verification of insurance benefits. Be aware that verification is a quote and not a guarantee of payment.
Some insurance plans might have limitations including co-pays, referrals from the Primary Care Provider, therapy visit limitations per calendar year, pre-certification, and deductible and co-insurance obligations. Should insurance payments be delayed, reduced, or denied, we will expect prompt payment from the responsible party. As a convenience, we accept checks, Visa, MasterCard, American Express and Discover.
If you choose not to utilize your insurance or do not have insurance coverage, please contact your local office regarding payment options including an administrative discount for payments made at the time of service.
